Job Overview
We are seeking a dedicated and passionate Dementia Health Coach to join our team. The ideal candidate will play a pivotal role in guiding individuals towards healthier lifestyles by providing education, support, and motivation. This position requires a strong understanding of public health principles and the ability to communicate effectively with diverse populations. The Health Coach will work closely with clients to assess their needs and develop personalised health plans that promote overall well-being.

Duties

Support clinets who are at different stages with dementia.
Conduct one-on-one coaching sessions to assess client goals and progress.
Develop tailored health plans based on individual needs and preferences.
Stay informed about current trends in public health and medical terminology to provide accurate information.
Collaborate with healthcare professionals, including nurses and GP to ensure comprehensive care for clients.
Monitor client progress and adjust health plans as necessary to ensure optimal outcomes.
Maintain accurate records of client interactions and progress reports.

Skills

An understanding of dementia.
Excellent spoken and written English.
Ability to show compassio, empathy and understanding to people effect with dementia.
Effective interpersonal skills.
Excellent organisation skills.
Ability to motivate and inspire clients towards achieving their health goals.
Able to work independently and proactively whilst at the same time demonstrating strong team work skills.
Understanding of the roles with the NHS and VCSE sector.
