3 Hospital Discharge Support Workers Immediate start
Age UK Leicester Shire & Rutland has partnered with the NHS to deliver a Hospital Discharge Support Service for patients transitioning from hospital to home. The service will run for an initial 6-week period, and success could lead to an extension. This is an exciting opportunity to make a real impact on patients’ lives during their recovery. A background in patient care or patient transportation would be advantageous.
Responsibilities:
- Transporting patients from the hospital to their homes, providing comfort and support along the way.
- Preparing homes to be welcoming, ensuring heating is on and the environment is safe.
- Collecting shopping and prescriptions to ensure patients have essential items upon discharge.
- Helping with light housework and other practical tasks to ease the transition.
- Providing emotional support to patients and their families, addressing any concerns.
Requirements:
- Experience in healthcare, social care, or a related field is helpful but not essential.
- Strong communication and practical support skills.
- A driver’s license and access to a vehicle are required for transporting patients.
- A compassionate, patient-centered approach.

Location: Leicester, Leicestershire & Rutland
Hours: Part Time 3 POSITIONS
· Monday 30 hours – Friday 8am - 2pm
· Monday 30 hours – Friday 2pm - 8pm
· Saturday/Sunday 15 hours – 10am - 6pm
Salary range : £11.80 – £12.30 - £13.30
